Key Responsibilities- Client Management: Conduct thorough assessments to understand the unique needs and preferences of each individual, and develop and implement personalized care plans that address medical, emotional, and social requirements.
- Induction and Training: Organize comprehensive induction programs for new support workers, covering company policies, procedures, and best practices, and ensure that new hires understand the specific needs of the individuals they will be supporting.
- Team Supervision: Provide ongoing supervision and mentorship to support workers, fostering a positive and productive work environment, and conduct regular performance reviews and provide constructive feedback to help team members improve and grow.
- Communication and Reporting: Schedule and attend regular meetings with clients, their families, and integrated care boards (ICB) contacts, communicate effectively to ensure all parties are informed and involved in care decisions, and prepare detailed reports on the progress and status of the individuals we support.
- Incident Management: Take prompt action to manage and resolve any incidents that occur, ensuring the safety and well-being of all involved, and conduct thorough investigations to determine the cause of incidents and prevent future occurrences.
- Experience: Previous experience of managing clients within the community, with a passion for delivering excellent client service.
- Skills: Excellent communication skills, ability to organize a busy workload and prioritize effectively, and proficiency in MS Office.
- Education: NVQ Level 3 (or equivalent) in Health & Social Care or a related topic, or be willing to study towards this.
- Other: A full UK driving licence and access to your own vehicle.
