About the Role

We are seeking a highly skilled and compassionate Crisis Care Practitioner to join our Crisis Assessment and Treatment Team (CATT) at West London Mental Health Trust.

Key Responsibilities
  • Conduct comprehensive assessments and provide brief home-based treatment to individuals with mental health problems to prevent hospital admission.
  • Work collaboratively with patients, families, and other healthcare professionals to develop personalized care plans.
  • Deliver the gatekeeping function for admission to inpatient mental health units and provide in-reach work in wards to facilitate discharges.
  • Plan, implement, and evaluate programs of care for clients in crisis with acute mental health problems in the community.
  • Coordinate activities with other professionals and agencies to ensure seamless care delivery.
Requirements
  • Strong clinical skills and experience in mental health assessment and treatment.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
  • Knowledge of mental health legislation and policies.
  • Ability to work collaboratively as part of a multidisciplinary team.
