Our mission at the Stroke Association is to empower stroke survivors and their families to live independent lives in the community.

We are seeking a highly skilled and passionate individual to join our team as a Community Connect Coordinator. This exciting role will involve supporting new stroke survivors and their carers to live independent lives in the community, providing personalised information, advice and support, and working collaboratively with Social Care, NHS, 3rd sector colleagues and other areas of the community to make a difference in the lives of people affected by stroke.

The successful candidate will be responsible for:

  • Supporting stroke survivors and their carers to access community resources and services.
  • Providing information and advice on stroke recovery and community living.
  • Working with volunteers to deliver community-based support services.
  • Promoting stroke-friendly communities and advocating for the needs of stroke survivors.

This is an excellent opportunity for someone who is passionate about making a difference in the lives of stroke survivors and their families. If you are a motivated and organised individual with excellent communication skills, we would love to hear from you.
