Job Title: Band 5/6 Mental Health Practitioner

We are seeking a skilled Mental Health Practitioner to join our Crisis Resolution Home Treatment Team. As a key member of our team, you will play a vital role in delivering high-quality care and support to individuals experiencing mental health crisis.

About the Role

You will work closely with our multidisciplinary team to provide assessments, advice, and support to individuals with mental health needs and learning disabilities.

Key Responsibilities
  • Conduct comprehensive assessments of individuals with mental health needs and learning disabilities
  • Develop and implement personalized care plans to support individuals in crisis
  • Work collaboratively with our team to provide high-quality care and support
  • Participate in ongoing training and professional development to enhance your skills and knowledge
Requirements
  • Registered Mental Health Nurse or Allied Health Professional
  • Experience of working with individuals with mental health needs and learning disabilities
  • Strong communication and interpersonal skills
  • Ability to work flexibly and respond to changing situations
