Community Based Care Team Lead

We are seeking a talented Adult Social Worker to join our Community Based Care team in Dorset. As a key member of our Home First team, you will play a vital role in promoting independence and achieving excellent outcomes for our service users.
Key Responsibilities:- Undertake strengths-based assessments and support planning to promote independence.
- Work collaboratively with a busy team and other local authority teams to achieve excellent outcomes.
The ideal candidate will have at least 2 years of field experience and excellent communication skills. We offer regular supervision, a robust learning and development offer, and opportunities for career progression.
Estimated Salary: £28,000 - £34,000 per annum
