About the Role

We are seeking a highly skilled and dedicated individual to join our team as an Early Intervention Psychosis Practitioner. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals experiencing a first episode of psychosis.

Key Responsibilities
  • Assessment and Intervention: Conduct comprehensive assessments of service users, develop and implement individualized care plans, and provide a range of interventions to support recovery from psychosis.
  • Care Coordination: Provide care coordination services for an agreed number of service users and their families, ensuring seamless transitions and continuity of care.
  • Collaboration and Partnership: Work closely with other locally based services and agencies to promote and develop a comprehensive Early Intervention in Psychosis Service, enhancing links with youth-focused services.
  • Record Keeping and Data Management: Maintain accurate records and input data into information systems within agreed time-scales, ensuring compliance with organizational standards.
