About the Role

We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at Claimont Health LTD. As a Mental Health Support Worker, you will play a vital role in providing person-centered care to our clients in the comfort of their own homes.

Key Responsibilities
  • Provide high-quality support to clients with mental health conditions, including ADHD, EUPD, and diabetes.
  • Work collaboratively with clients, families, and carers to develop and implement personalized care plans.
  • Ensure that all aspects of client treatment and progress are accurately recorded and maintained.
  • Participate in regular team meetings and contribute to the development of the team's clinical practice.
  • Adhere to Claimont's policies and procedures, including those related to confidentiality, data protection, and safeguarding.
Requirements
  • Minimum 12 months experience as a Mental Health Support Worker or equivalent.
  • Basic computer skills and ability to maintain accurate records.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability in a fast-paced environment.
