Job Description:

We are seeking a skilled Physiotherapist to join our team as a Community Rehabilitation Specialist. As a key member of our multidisciplinary team, you will work closely with Therapy Assistants, Physiotherapists, and Occupational Therapists to provide high-quality rehabilitation services to patients in the community.

Key Responsibilities:

  • Deliver community-based rehabilitation services to patients with a range of physical and functional impairments
  • Work collaboratively with the multidisciplinary team to develop and implement individualized rehabilitation plans
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Develop and implement effective treatment plans to improve patient outcomes
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination

Requirements:

  • Registered Physiotherapist with a strong background in community rehabilitation
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong assessment and treatment planning skills
  • Knowledge of community-based rehabilitation principles and practices
