- Providing emotional and practical support to enable individuals to maintain their independence and autonomy
- Helping individuals develop new skills and accessing community activities and resources
- Promoting physical and mental well-being through personalized care plans and interventions
- Collaborating with colleagues to ensure seamless care transitions and effective communication
