Adult Services Team Social Worker

We are seeking a skilled and compassionate Adult Services Team Social Worker to join our team at Randstad Care Careers. As a key member of our Adult Services Team, you will play a vital role in supporting adults requiring care and assistance.
Key Responsibilities:
- Conduct thorough assessments of adults' needs, ensuring their care plans are tailored to their individual requirements.
- Develop and maintain effective support plans, promoting independence, well-being, and long-term support for service users.
- Collaborate with multidisciplinary teams, service users, and their families to ensure seamless care delivery and adherence to statutory requirements.
- Monitor and review support plans, making adjustments as needed to ensure optimal outcomes.
- Safeguard and promote the welfare of adults, adhering to relevant legislation and policies.
Requirements:
- Proven experience in adult social care, with a strong understanding of the Care Act 2014.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.
- Ability to work autonomously, prioritizing tasks and managing multiple responsibilities.
- Strong analytical and problem-solving skills, with the ability to think critically and make informed decisions.
