Job Summary

We are seeking a highly skilled and motivated nurse to join our established multidisciplinary diabetes team. The ideal candidate will have a strong interest in diabetes and a passion for delivering high-quality care to patients and healthcare professionals.

The successful candidate will be an excellent communicator and educator, with good organisational skills and the ability to work well as part of a team. They will also have a strong desire to gain new skills and knowledge, and be committed to providing high standards of care to patients with diabetes.

The post-holder will provide evidence-based diabetes care and advice to patients with diabetes, with responsibility for a patient caseload. They will also oversee the Diabetes Virtual Ward, identifying and onboarding suitable patients, and liaising with colleagues across the primary-secondary care interface to support safe and efficient diabetes management and transfer of care.

The post-holder will disseminate diabetes education to patients and healthcare professionals across the Trust as part of regular staff training programmes. They may also be asked to work flexibly within a 6-day working week in conjunction with other Diabetes CNS colleagues.

Main Duties of the Job

The post-holder will be responsible for:

  • Providing evidence-based diabetes care and advice to patients with diabetes
  • Overseeing the Diabetes Virtual Ward, including identifying and onboarding suitable patients
  • Liaising with colleagues across the primary-secondary care interface to support safe and efficient diabetes management and transfer of care
  • Disseminating diabetes education to patients and healthcare professionals across the Trust
