Care Transformation Specialist

Job Overview:
Derbyshire County Council is seeking a skilled Care Transformation Specialist to join their Adult Social Care Transformation team. The successful candidate will work on reviewing care packages for individuals with learning disabilities and autism, ensuring they receive the best possible support.
Responsibilities:
- Reviewing and reassessing care packages for people living in specialist residential or supported living placements to ensure levels of support are appropriate and least restrictive.
- Involved in reviewing and reassessing care packages for people with learning disabilities and autism who live in high-cost supported living schemes throughout the county.
- Reducing levels of commissioned support in specialist residential or nursing placements as a result of reassessment of eligible needs under the Care Act 2014.
- Ensuring timely applications for Continuing Health Care and providing support around DST reviews.
Requirements:
The ideal candidate will be able to travel when required and will have strong communication skills. This role offers a great opportunity to make a difference in the lives of individuals with learning disabilities and autism.
