About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our dynamic team as a Clinical Nurse Specialist. This is an exciting opportunity to work with vulnerable individuals and make a meaningful difference in their lives.

Key Responsibilities
  • Conduct comprehensive assessments of individuals' care needs, using the National Framework for NHS Continuing Healthcare as a guide.
  • Develop and implement personalized care plans, working collaboratively with healthcare professionals and individuals to ensure the best possible outcomes.
  • Provide expert advice and guidance on complex healthcare issues, influencing best practice and contributing to the development of training programs.
  • Work closely with the Senior Nurses to process Funded Nursing Care and Continuing NHS Health Care Applications, ensuring seamless implementation of the National Framework.
  • Collaborate with the All Age CHC Service and BSOL ICB to deliver high-quality care management processes, ensuring value for money and effective use of resources.
Person Specification
  • Registered Nurse with a current NMC registration or equivalent.
  • Previous experience of undertaking Continuing Care Assessments and developing care plans.
  • Conversant with national and local guidance on NHS Continuing Health Care/NHS Funded Care for Adults Continuing Healthcare and with detailed understanding of the correct use of the national tools (checklist, DST, Fast Track).
  • Extensive nursing skills in Adults with complex healthcare needs, with the ability to analyze and interpret quantitative and qualitative data.
  • Ability to plan and implement internal policies, processes, and procedures, with a strong understanding of Commissioning arrangements.
