Job Description

The successful applicant will work with young people aged 16-26 years who are leaving care in Highland, supporting them to achieve their full potential. The role involves assessing need and coordinating a plan of support for young people, offering advice and guidance to enable them to work towards agreed outcomes to aid the transition to independence.

Responsibilities:
  • Assessing the needs of young people and developing individualized plans to support their transition to independence
  • Providing advice and guidance to young people on issues such as education, employment, and housing
  • Collaborating with other professionals to ensure that the needs of young people are met
  • Keeping accurate records of progress and outcomes
Requirements

We are looking for an individual with strong assessment skills and experience in report writing and recording. You should have a good understanding of the challenges faced by young people leaving care and be able to provide holistic support to meet their individual needs.

The estimated salary for this role is £25,000-£30,000 per annum, depending on experience.
