Job Description

We are seeking a highly motivated and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in supporting individuals to maintain their independence, make informed decisions, and achieve their goals.

Key Responsibilities

  • Provide person-centred support to individuals, promoting their autonomy and well-being
  • Develop and maintain positive relationships with service users, their families, and other stakeholders
  • Contribute to the creation and implementation of individual support plans, ensuring they are tailored to meet the unique needs of each service user
  • Participate in regular supervisions and training to enhance your skills and knowledge
  • Collaborate with the team to ensure a seamless and effective service delivery
