Job Description

We are seeking an experienced Rehabilitation Assistant Practitioner to join our team at NHS Lanarkshire.

The successful candidate will work under the supervision of registered therapy staff to assist patients in achieving their rehabilitation goals.

About the Role

This permanent, full-time position is based in Lanarkshire within Hospital at Home and involves working 37 hours per week.

Responsibilities

  • Undertake baseline assessments within clearly defined parameters for the identified service.
  • Implement, adapt, and progress treatment programmes within agreed protocols and defined parameters.
  • Deliver flexible, patient-centred programmes across various clinical settings and discharge or transition care within agreed protocols and defined parameters.

Required Skills and Qualifications

  • HNC in healthcare, education, or social care or SVQ III in healthcare, education, or social care or equivalent experience in a health, education, or social care environment.
  • Awareness of health legislation relevant to the clinical area.
  • Knowledge of the therapeutic process, techniques, and/or activities relevant to the client group.
  • Basic assessment skills.
  • Ability to apply a range of technical and practical rehabilitation skills.
  • Effective listening, communication, and interpersonal skills.
  • Time management, planning, and organisational skills.
  • Working knowledge of basic information technology.
  • Ability to work individually or as part of a team.
  • Car Driver with a full, valid UK/EU/EEA licence.

Estimated Salary: £25,000 - £30,000 per annum, depending on experience.
