We are recruiting for a skilled Adult Social Worker to join our Sensory, Physical and Neurological team in Warrington.
Job Summary:
As a qualified Adult Social Worker, you will work closely with our team to provide high-quality care and support to adults with sensory, physical, and neurological needs.
Key Responsibilities:
- Provide person-centered care and support to adults with sensory, physical, and neurological needs
- Work collaboratively with the multidisciplinary team to develop and implement care plans
- Conduct assessments and reviews to ensure the best possible outcomes for service users
- Maintain accurate and up-to-date records
Requirements:
- Qualified Social Worker with experience in sensory, physical, and neurological care
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Knowledge of relevant legislation and policies
