At Social Personnel, we are seeking a highly skilled and compassionate Personal Advisor to join our team. As a Personal Advisor, you will play a vital role in supporting young people who have left care to make a successful transition into adulthood.
Key Responsibilities:- Deliver a high-quality service to young people who were formerly accommodated, ensuring good outcomes and enhancing their life chances.
- Provide support, advice, and assistance to enable young people to maximize education, training, work, and accommodation opportunities.
- Work with care leavers with multiple and complex needs, including social exclusion, poor education attainment, vulnerability to exploitation, and unstable family relationships.
- Write reports, maintain up-to-date records, and attend meetings related to young people's services.
- Arrange and chair Personal Educational Plans for statutory school-age young people.
- Motivate and engage young people in developing and implementing pathway plans.
- Work in cooperation with statutory and voluntary sector agencies and professionals to ensure effective coordination and provision of services.
- Advocate on behalf of young people and liaise with other agencies and professionals regarding their accommodation, health, employment, training, immigration, and other relevant issues.
- Monitor the welfare of children and young people, including child protection and safeguarding issues.
- Carry out assessments of independent living skills and support young people to develop practical and emotional skills for independent living.
