Key Responsibilities:- Provide comprehensive nursing care to patients with a variety of long-term health conditions
- Conduct health promotion and screening activities to support patients in maintaining good health
- Administer vaccinations and medications as required
- Conduct wound care and provide support for patients with chronic conditions
- Work collaboratively with the wider team to deliver excellent nursing care and maximize practice income
- Registered with the NMC
- Degree in Nursing
- Substantial Practice Nurse experience
- Evidence of ongoing CPD
- Minimum of 1 Long Term Condition diploma
- Knowledge of patients with long-term conditions
- Familiarity with Patient Group Directions (PGD's) and associated policy
- NHS Pension with 20.6% Employer Contribution
- 25 days / 200 hours / 5 weeks annual leave plus Bank Holidays (pro-rata, if part-time)
