Main Duties of the Job

The Living Well service aims to empower individuals to live well by equipping them with skills and knowledge to achieve their goals and live day-to-day with a sense of purpose and wellbeing.

We're looking for an individual with excellent interpersonal and communication skills, a team player committed to collaborative working with colleagues and patients to promote self-care. The successful candidate will demonstrate innovative and confident teaching skills and an ability to assess patients' health and psychosocial needs to plan appropriate care through shared decision making.

Key Responsibilities

  • Coordinate and develop the Living Well programme of sessions, activities and courses
  • Arrange and co-ordinate online/remote support
  • Assess patients' health and psychosocial needs to plan appropriate care
  • Develop and deliver wellbeing-focused support to our caseload
