Job Summary:

We are seeking a highly motivated and experienced Long-term Intervention Specialist to join our team in Cumbria. The successful candidate will have excellent communication skills and the ability to work independently and as part of a team.

About the Job:

  • Respond to requests for assessments, support planning and reviews, including hospital discharge planning
  • Work alongside health colleagues locally to reduce the risk of hospital admissions
  • Develop and implement effective care plans to meet the needs of service users

Requirements:

  • Degree in Social Work or related field
  • Experience in long-term interventions and case management
  • Excellent communication and interpersonal skills

Salary and Benefits:

Salary: £34 per hour (umbrella)
