We are seeking a dedicated Band 6 Community Psychiatric Nurse (CPN) to join our team in Reading, providing care coordination and support to individuals aged 18 to 65.

Job Summary:

This is an exciting opportunity to work in a dynamic and supportive team environment, contributing to the delivery of high-quality mental health services.

About the Position:
  • Position: Band 6 CPN Adult CMHT
  • Location: Reading area
  • Contract: Full-time (Monday to Friday, 9:00 am - 5:00 pm)
  • Rate: GBP26.50 per hour
  • Duration: Start ASAP, ongoing for a minimum of 3 months
Responsibilities:
  • Assess and develop care plans for clients with varying mental health needs.
  • Provide individualized care coordination and support to clients and their families.
  • Work collaboratively with multidisciplinary teams to ensure comprehensive care.
  • Conduct community visits as required, ensuring a person-centred approach.
