About the Role

We are seeking a highly skilled and dedicated Senior Practitioner to join our Community Mental Health Team. As a key member of the team, you will be responsible for providing high-quality care to older adults with complex mental health needs.

The successful candidate will have significant experience working with people with mental ill health in a care environment, with a strong focus on clinical supervision and leadership. You will be able to demonstrate excellent assessment and analysis skills, as well as interpersonal, communication, and team working skills.

Key Responsibilities
  • Provide assessments, coordination of care, and interventions in the community for individuals with complex mental health needs.
  • Work collaboratively with a range of statutory and third sector agencies to promote holistic wellbeing and recovery-focused care.
  • Develop and maintain effective relationships with service users, carers, and other professionals to ensure seamless care pathways.
  • Contribute to the development and implementation of care plans, ensuring they are tailored to the individual's needs and goals.
Requirements
  • Post-graduate qualification in a relevant area, such as mental health nursing, occupational therapy, or social work.
  • Significant experience working with people with mental ill health in a care environment.
  • Excellent assessment and analysis skills, with the ability to develop and implement effective care plans.
  • Strong interpersonal, communication, and team working skills, with the ability to work collaboratively with a range of professionals.
