Key Responsibilities

  • Support clients from beginning to end of their time with Home Instead, ensuring their needs are met and exceeded.
  • Conduct quality assurance visits to clients and their families to ensure high standards of care.
  • Introduce new Care Professionals to clients and complete all necessary documentation.
  • Assist in creating, updating, and auditing client care plans to ensure they meet the client's needs.
  • Ensure clear risk assessments are completed for clients and Care Professionals.
  • Use our care management software to audit daily care logs and medication records.
  • Participate in an out-of-hours on-call rota to ensure 24/7 support for clients.
  • Complete the first care visit for new clients to ensure their care plans meet their needs.

New Client Care

  • Attend care consultations to understand client needs and preferences.
  • Complete care plans that meet the client's needs and exceed their expectations.
  • Prepare all necessary documents for new clients, including care plans and introductions.
  • Introduce new Care Professionals to clients and complete all necessary documentation.
  • Ensure client files are complete and up-to-date.
  • Conduct new client quality assurance visits to ensure high standards of care.

Existing Client Care

  • Introduce new Care Professionals to existing clients and complete all necessary documentation.
  • Prepare for and conduct quality assurance visits to ensure high standards of care.
  • Prepare for and conduct service review visits to ensure client needs are met.
  • Identify opportunities to improve client outcomes and provide excellent customer service.
