Working Hours: Days, afternoons & nights over a 7-day week, including weekends and bank holidays
Job Purpose: To provide care and support to individuals with a learning disability in a short break setting.
Main Responsibilities:
- Follow individual care plans and risk assessments to ensure the well-being and safety of service users.
- Contribute to the monitoring and assessment of service users' plans, reviews, and assessment of risks, ensuring compliance with organisational policies and procedures in line with Care Quality Commission Essential Standards of Quality and Safety.
- Work with the team to provide sensitive care and guidance to service users in areas of personal care, daily living, health, social integration, and emotional support.
- Develop and maintain effective relationships with service users, their families, and other stakeholders.
- Participate in the development and implementation of Person-Centred Plans that meet individual needs.
- Monitor and review service users' progress, making recommendations for improvements as necessary.
- Collaborate with other teams and departments to ensure a seamless service delivery.
Requirements:
- Experience of working with individuals with learning disabilities.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work a rota system, including evenings, weekends, and bank holidays.
- Good IT skills and ability to use electronic records.
- Ability to write clear and concise reports and records.
