Person-Centered Support Specialist Job Description

We are seeking a highly skilled and empathetic Person-Centered Support Specialist to join our team at Harley House.

The successful candidate will be responsible for providing person-centered support to enable individuals to lead fulfilling lives.

Main Responsibilities:

  • Deliver person-centered support to meet the individual's specific needs and goals
  • Develop and maintain effective relationships with individuals, families, and healthcare professionals
  • Collaborate with colleagues to ensure seamless care and support services
  • Ensure accurate and timely recording of care records

Requirements:

  • Highly skilled and empathetic individual with excellent communication and interpersonal skills
  • Able to work independently and as part of a team
  • Flexibility to work varied shifts, including evenings and weekends
  • Basic computer skills, including Microsoft Office
