Discharge Coordinator

About the Role
We are seeking a skilled Discharge Coordinator to join our team at Nottingham University Hospitals NHS Trust. As a key member of our multi-disciplinary team, you will play a vital role in ensuring the smooth transition of patients from our care to their next destination.
Key Responsibilities
- Act as the primary point of contact for discharge information, providing timely and accurate updates to patients, relatives, and carers.
- Collaborate with ward teams, hospitals, and trust bed coordinators to coordinate patient transfers and ensure seamless care transitions.
- Develop and maintain effective relationships with patients, families, and healthcare professionals to ensure comprehensive discharge planning.
- Contribute to the development and implementation of discharge protocols and procedures to improve patient outcomes and satisfaction.
Requirements
- Demonstrate excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Proven experience in a healthcare setting, with a strong understanding of patient confidentiality and data protection.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
- Ability to work independently and as part of a team, with a flexible approach to working hours and shifts.
