About the Role

We are seeking an experienced Care Pathway Facilitator to join our team at Barking, Havering and Redbridge University Hospitals NHS Trust. As a key member of our Geriatric Division, you will play a vital role in ensuring the smooth discharge of patients from our Queens Frailty Unit.

Key Responsibilities
  • Collaborate with the team of patient pathway coordinators to facilitate discharges and ensure timely and safe patient transfers.
  • Work closely with nursing staff and medical teams to develop and implement effective discharge plans.
  • Communicate effectively with patients, relatives, and carers to ensure their needs are met and concerns are addressed.
  • Maintain accurate records and documentation to support the discharge process.
  • Contribute to the development and implementation of care pathways and protocols to improve patient outcomes.
Requirements
  • Proven experience in an acute hospital clinical role, preferably in elderly care.
  • Excellent interpersonal and communication skills, with the ability to work effectively with diverse stakeholders.
  • Computer skills, including Microsoft Word and Excel, with the ability to learn new data technology.
  • GCSE standard or equivalent in English, with the ability to communicate complex information effectively.
