Palliative Care Specialist Occupational Therapist

We are seeking an experienced Occupational Therapist to join our specialist team at St Lukes Hospice. The successful applicant will work predominantly within the specialist community team, providing high-standard Occupational Therapy assessments with patients in their own homes, alongside the Clinical Nurse Specialist team.

The AHP team offers an in-reach service to patients admitted to the inpatient Unit, assisting in discharge planning and therapeutic interventions. The post holder will need to be able to work autonomously and be responsible for complex holistic assessments and planning care to meet patients' practical, psychological, spiritual, and social needs.

The successful applicant will work closely with primary and secondary care professionals and Hospice colleagues, requiring good communication and team-working skills. This is a challenging yet rewarding role, helping to make a real difference to the quality of life and levels of independence for those with life-limiting conditions.

Requirements:

  • Experience as an Occupational Therapist in a palliative care setting
  • Ability to work autonomously and as part of a team
