We are seeking an experienced Social Care Specialist to join our team at Hertfordshire Partnership University NHS Foundation Trust. As a key member of our multi-disciplinary team, you will be responsible for providing high-quality social care services to adults experiencing mental ill-health.

About the Role:

  • Assess and plan care for individuals with complex needs, focusing on recovery, choice, control, and independence.
  • Collaborate with service users, carers, and other professionals to deliver person-centred care plans.
  • Champion adult safeguarding practices and ensure the protection of vulnerable adults and children.
  • Contribute to the development of comprehensive assessments and reviews, identifying risks and complexities.

Your Skills and Qualifications:

  • Professional social work qualification (e.g., CQSW, DipSW, or BA/BSc in Social Work)
  • Registration with Social Work England
  • Training in outcomes-focused assessments, personalisation, and safeguarding
  • Strong post-qualification experience in social work, particularly in community mental health
  • Excellent communication and analytical skills, with experience working with electronic recording systems
