Elderly Care Specialist

This is an exciting opportunity to join our team as a GP with specialist interest in frailty and virtual ward. We are looking for a skilled medical professional who can provide acute care for older people living with frailty, safely and conveniently in their own homes.
Job OverviewThe post holder will work within our developing 'Hospital at Home' service, providing medical care at home for patients who would otherwise require hospital admission. This role requires strong clinical skills, excellent communication abilities, and the capacity to work effectively within a multidisciplinary team.
Responsibilities- To work as part of the multi-disciplinary team to support and provide a comprehensive Geriatric Service within the Trust.
- Maintenance of the highest clinical standards in the management of patients in their own place of residence.
- Assisting with teaching, training, and assessment of junior staff, nursing staff, and other members of the multi-disciplinary team.
- Active participation in departmental and Trust matters concerning Clinical Governance and audit.
- Ensuring active participation in continuing medical education (CME).

We are seeking a candidate with:
MRCGP qualification.Diploma in Geriatric Medicine or equivalent experience.Evidence of experience working in a multidisciplinary team and effective team working skills.12 years post-medical degree experience, including 6 years with specific experience in elderly medicine.Evidence of leadership and management training (desirable).Working ArrangementsThe successful candidate will be employed on a full-time basis, with a salary of approximately £95,000 - £110,000 per annum, depending on experience.
