Exciting Opportunity for an Experienced Nurse

We are seeking an experienced nurse to join our Heart Failure Team on a secondment for 18 months. The successful candidate will work across multiple sites, including UHCW, Hospital of Saint Cross, Coventry Community, and home visits. A car and driving license are essential for this role.

Key Responsibilities
  • Recommend evidence-based therapies for patients with heart failure, including medications and cardiac devices.
  • Review patients in face-to-face and virtual clinics, virtual ward, in hospital, at home, and via telephone to support patients and carers with stable and unstable heart failure.
  • Contribute to developing robust guidelines and protocols for heart failure care and provide clinical advice and education to ensure seamless pathways of care.
  • Support advanced care planning strategies for end-stage heart failure patients.
