Role Overview
We are seeking highly skilled Care Specialist Coordinators to join our team in Islington, working within our 7-day rapid service for hospital discharges and community settlement.
The ideal candidate will have experience in hospital discharge work, completing care Act Assessments, and arranging urgent care. This role requires strong organisational skills, excellent communication, and a passion for delivering high-quality care.
Key Responsibilities
- Coordinating the discharge process from hospitals to community settings.
- Completing comprehensive care Act Assessments to identify individual needs.
- Developing and implementing personalised care plans to ensure seamless transitions.
- Maintaining effective communication with healthcare professionals, families, and individuals receiving care.
Requirements
To succeed in this role, you will need:
- A proven track record in hospital discharge or similar field.
- Excellent assessment and planning skills.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
