Job Description: This role offers a unique opportunity to contribute to the safe management of medication for patients in our primary care network. The successful candidate will be responsible for reconciling medications, collaborating with pharmacists, and providing feedback on clinical concerns. A strong understanding of primary care networks and evidence-based medicine is essential.

Key Responsibilities:
  • Reconcile medications for patients transferring back to primary care.
  • Collaborate with pharmacists to develop efficient systems for repeat prescribing medication.
  • Provide feedback on clinical concerns related to medication management.
  • Manage specialist prescription requests in line with local guidelines.


Requirements: To succeed in this role, you will need:
  • Theoretical and practical knowledge of Primary Care Networks and General Practice.
  • Evidence of continued professional development (CPD) and experience working as a qualified pharmacy technician.
  • Good clinical pharmacy knowledge and ability to maintain confidentiality.
