Advanced Nurse Practitioner

We are seeking a highly motivated and experienced Advanced Nurse Practitioner to join our dynamic team in a Primary Care setting.
About the Role:This is an exciting opportunity for a skilled and enthusiastic Advanced Nurse Practitioner to provide high-quality nursing care to our patients. As a key member of our clinical team, you will be responsible for delivering evidence-based practice, screening, assessment, treatment, and education to patients presenting with long-term conditions and acute problems.
Key Responsibilities:- Provide and maintain a high standard of nursing care to the practice population.
- Deliver evidence-based practice for patients presenting with long-term conditions and acute problems.
- Screen, assess, treat, and educate patients.
- Support the practice to maximise achievement against local and national targets.
- Registered Nurse with up-to-date NMC Registration.
- Experience in a Primary Care setting.
- Knowledge of current policies, protocols, and national targets.
- Clinical skills including Chronic Disease Management, Smear taking, Child and adult immunisations, Women's Health, Wound Management, Health Promotion, Men's Health, Cardiovascular Disease Management, and Travel Health.
- Independent Prescribing.
- Minor Illness.
- Knowledge of Vision computer software.
- Permanent position.
- Full-time or part-time available.
