About the Role

We are seeking a highly skilled and compassionate Advanced Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As a Liaison and Diversion Specialist, you will play a vital role in supporting individuals with complex needs within court and custody settings.

Key Responsibilities
  • Provide holistic support to individuals, families, and friends, facilitating access to healthcare services and addressing practical problems affecting their health and wellbeing.
  • Act as an empathetic advocate, promoting the rights and responsibilities of service users, while maintaining professional boundaries.
  • Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and law enforcement agencies, to ensure seamless care coordination.
  • Develop and maintain effective relationships with service users, families, and carers, promoting trust and confidence in the healthcare system.
  • Contribute to the development of care plans, ensuring that individual needs are met and that service users receive the best possible outcomes.
Requirements
  • NVQ level 3 in Health and Social Care or equivalent qualification.
  • Minimum of 12-18 months' experience in a relevant healthcare setting, preferably in a Liaison and Diversion role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Knowledge of the criminal justice system, Liaison and Diversion agenda, and healthcare services.
  • Ability to work autonomously, with strong organizational and time management skills.
