Job Summary:

Creative Support is seeking a highly motivated and caring individual to join our team as a Support Coordinator in our Doncaster Supported Living Services. As a Support Coordinator, you will play a vital role in providing outcome-focused support to enable clients to lead fulfilling and valued lives.

Key Responsibilities:

  • Provide support to clients to maintain their tenancies, participate in their local community, and engage in activities that promote their physical and emotional well-being.
  • Effectively meet the needs of clients and take additional responsibilities such as rota management, induction of staff, and staff supervision.
  • Work in a flexible manner, including working evenings and weekends, and participate in our local On Call system.
  • Administer medication and provide support with daily living tasks.

Requirements:

  • A clean, up-to-date driving license and access to a vehicle for traveling around the Doncaster area.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, staff, and other stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to work.
  • Knowledge of the principles of person-centered support and the ability to apply these in practice.
