We are seeking an experienced Advanced Clinical Practitioner to join our Clinical Hub team in the West Midlands region. As a key member of our team, you will have the opportunity to bring your advanced nursing skills to an organization dedicated to making a difference in the lives of others.

About the Role

This is a full-time position, working Monday to Friday. You will be responsible for providing clinical leadership and expertise to our multidisciplinary team, working collaboratively to deliver high-quality patient care. Your responsibilities will include:

  • Completing comprehensive geriatric assessments, diagnosis, treatment, and evaluation of care.
  • Communicating and working collaboratively with the interdisciplinary teams across sites to support the delivery of the Frailty framework.
  • Providing clinical education, leadership, and upskilling the local workforce.

Requirements

To be successful in this role, you will need:

  • Registration as a Registered Nurse Adult, Mental Health with current NMC registration or Registered Allied Healthcare Professional with current HCPC registration.
  • Independent or supplementary prescriber qualification.
  • MSc in Advanced Practice or relevant practice experience.
  • Completion of British Geriatric Society Tier 3 Frailty or willingness to work towards.
