We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Somerset Healthcare and Rehabilitation Center. Job Responsibilities:
  • Provide nursing care to patients, including administering medications and treatments as assigned.
  • Prepare for and assist physicians and other healthcare professionals with diagnostic procedures and treatments.
  • Reinforce health care instructions to patients and their families, promoting a smooth transition to home care.
  • Participate in planning, implementing, and evaluating nursing care and other patient activities, ensuring a comprehensive approach to patient care.
  • Assist with patient admission, transfer, and discharge, maintaining a seamless experience for our patients.
  • Monitor and report patients' vital signs, symptoms, and general condition to the appropriate healthcare professionals.
  • Record patient observations and responses to nursing care, maintaining accurate and up-to-date patient charts.
  • Maintain a safe and comfortable environment for patients, adhering to infection control and safety protocols.
