Job Title: Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to join our team at Improving Health LTD. As an Advanced Nurse Practitioner, you will play a vital role in providing high-quality patient care in a dynamic and forward-thinking primary care setting.

Job Summary:

The successful candidate will be responsible for providing advanced nursing care, assessment, and treatment to patients of all age groups. This will include conducting thorough assessments, ordering and interpreting diagnostic tests, and developing and implementing evidence-based treatment plans. You will also be responsible for conducting home visits to patients who are unable to attend the practice due to mobility issues, chronic illness, or other medical reasons.

Main Responsibilities:
  • Conduct thorough assessments of patients' health conditions, including physical examinations, medical histories, and symptom evaluation.
  • Order and interpret diagnostic tests and investigations.
  • Develop and implement evidence-based treatment plans for patients, including prescribing medications, administering treatments, and providing health education.
  • Conduct home visits to patients unable to attend the practice, assessing their healthcare needs and providing appropriate care.
  • Coordinate with the practice team to schedule and prioritize home visits efficiently.
  • Identify risks and make necessary arrangements to ensure the safety and well-being of both patients and healthcare providers during home visits.
  • Document and communicate findings, interventions, and care plans resulting from home visits to ensure continuity of care.
  • Collaborate with the general practitioner (GP) and other healthcare professionals to manage complex or challenging cases.
  • Provide consultations to patients, offering advice, information, and support to promote self-management of conditions.
  • Participate in multidisciplinary team meetings, contributing expertise and insights to optimize patient outcomes.
  • Educate patients and their families about health promotion, disease prevention, and self-care techniques.
  • Provide guidance on managing chronic conditions and promoting a healthy lifestyle.
  • Offer counselling and support for emotional well-being, mental health concerns, and lifestyle modifications.
  • Maintain accurate and up-to-date electronic health records for all patient interactions, including home visits.
  • Ensure adherence to data protection and patient confidentiality regulations.
  • Document interventions, treatment plans, and outcomes to facilitate effective communication and continuity of care.
Requirements:
  • Registered Nurse (RN) qualification with active registration with the Nursing and Midwifery Council (NMC) in the UK.
  • Advanced Nurse Practitioner qualification (ANP) at master's level or equivalent.
  • Additional qualification or certification in Independent Prescribing (V300) or willingness to obtain this qualification.
  • Proven experience as an Advanced Nurse Practitioner in a General Practice/Primary Care setting.
  • Demonstrated competence in conducting home visits and managing patients in their own homes.
  • Excellent clinical assessment, diagnostic, and decision-making skills.
  • Strong communication and interpersonal skills to establish rapport with patients, families, and multidisciplinary teams.
  • Ability to work autonomously, make independent judgments, and prioritize workload effectively.
  • Proficiency in using electronic health records (EHR) and other relevant software applications.
