Based in South West London, this exciting role offers the opportunity to work with a multidisciplinary team to assess individuals and determine their eligibility for fully funded NHS continuing healthcare or funded nursing care.

About the Role

We are seeking a skilled professional to manage referrals from various professionals, patients, and carers, determining eligibility for FNC or NHS CHC. As a key member of our team, you will be responsible for explaining the process clearly to patients, families, health and social care colleagues, and care providers.

Main Responsibilities
  • Determine eligibility for Fully Funded NHS Continuing Healthcare (CHC) or Funded Nursing Care (FNC) through assessments and reviews
  • Work collaboratively with multidisciplinary teams to establish eligibility and develop suitable care packages
  • Manage referrals from various sources, including professionals, patients, and carers
  • Communicate effectively with patients, families, and healthcare professionals to ensure transparency and understanding throughout the process
