Job Description: Adult Support Specialist

We are recruiting an Adult Support Specialist to join our team at Sanctuary Personnel Ltd. The successful candidate will be responsible for providing statutory support to adults with care and support needs and their carers/families. This role involves working in partnership with various stakeholders to deliver coordinated support and achieve positive outcomes.

About the Role:
This is an exciting opportunity to make a real difference in people's lives. As an Adult Support Specialist, you will be responsible for:
- Providing assessment, planning, and review processes for adults with care and support needs
- Developing and implementing support plans to meet individual needs and goals
- Working with other agencies and organizations to coordinate services and ensure seamless care delivery

Key Responsibilities:
- Assessing individual needs and developing support plans in collaboration with service users and stakeholders
- Coordinating services to ensure timely and effective delivery of care and support
- Monitoring and reviewing support plans to ensure they remain relevant and effective

Requirements:
- Social Work Qualification or equivalent
- Proven experience in adult social care, preferably in a specialist or coordinator role
- Strong communication and interpersonal skills, with the ability to build effective relationships with service users, families, and colleagues
- Ability to work autonomously, prioritizing tasks and managing competing demands

Working Hours:
- Full-time, 37 hours per week
- Flexible working arrangements may be considered
