Job Description

We are seeking a qualified pharmacist to join our team as a Pharmacist in Community Care. The successful candidate will work closely with our healthcare professionals and patients to provide expert advice and guidance on medication management in community settings.

You will be responsible for conducting regular reviews of medication prescriptions, identifying areas for improvement, and implementing changes to optimize patient care. You will also work closely with our community teams to ensure seamless transitions between hospital and community care.

The ideal candidate will have a strong background in pharmacy practice, excellent communication skills, and the ability to work independently. You will also have experience with IT systems and be comfortable working in a fast-paced environment.

In return, we offer a competitive salary of £55,000 - £75,000 per annum, depending on experience, plus a range of benefits including a generous pension scheme and opportunities for professional development.
