About the Role

Dorset HealthCare University NHS Foundation Trust is seeking a highly skilled MAS Specialist Community Practitioner to join our team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to individuals with dementia and their families.

Key Responsibilities
  • Perform initial assessments with formal cognitive testing to diagnose dementia
  • Offer clinic and home-based assessments, as well as NHS video consultations and phone assessments
  • Communicate effectively with clients and their carers, discussing diagnoses, treatment options, and care plans
  • Work closely with health, medical, and psychology colleagues to monitor anticholinesterase drugs and memantine
  • Liaise with colleagues in dementia services, primary care, community health, and social care services
  • Use a car to assess and monitor patients in their own homes and community settings
  • Deliver person-centred care while meeting local and national targets set by the NHS Dorset Clinical Commissioning Group and NHS England

Requirements
  • Registered practitioner to degree/diploma level with post-registration diploma level specialist training
  • Learning and Assessing in Practice Qualification or equivalent practice assessors training
  • Evidence of recent professional development in an up-to-date portfolio
  • Non-Medical Prescriber (if professionally appropriate)
Desirable Qualifications and Experience
  • Knowledge and understanding of Trust Strategy relevant to the role
  • Recent previous experience within a comparable role
