About the Role
We are seeking a skilled and compassionate Home Care Practitioner to join our team at North Cumbria Integrated Care NHS Foundation Trust. As a key member of our community team, you will work alongside patients and their families to provide high-quality care and support in their own homes.
Key Responsibilities
- Deliver person-centred care to patients, promoting independence and dignity at all times.
- Work collaboratively with the multidisciplinary team to develop and implement care plans that meet the individual needs of patients.
- Provide emotional support and reassurance to patients and their families, addressing any concerns or anxieties they may have.
- Contribute to the development and maintenance of a safe and supportive environment for patients, promoting their physical and emotional well-being.
- Participate in regular team meetings and contribute to the development of care plans and strategies to improve patient outcomes.
Requirements
- Hold a care certificate or be willing to undertake this.
- Have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
- Be able to work flexibly, including evenings, weekends, and bank holidays, as required.
- Be committed to delivering high-quality care and support, with a focus on patient-centred care and person-centred values.
