Hours: 37.5 hours per week across Kirklees, UK

Accountable To: Team Leader for Unplanned Care and Clinical Nurse Lead

Job Purpose:

We are seeking a dedicated and experienced Senior Physiotherapist to contribute to holistic assessments for adults with complex physical and medical needs. The primary goal is to work with key stakeholders and community services to avoid hospital admissions and assist in the timely discharge of those receiving care.

  • Manage clients over the age of 18 requiring support discharge from acute settings, particularly those with complex health and social needs.
  • Provide advanced physiotherapy assessments, treatment, and evaluation of clients' needs to avoid unnecessary hospital admissions and ensure swift, appropriate discharges.
  • Work within a multidisciplinary team (MDT) to avoid inappropriate admissions to long-term care.
  • Educate and support other healthcare professionals, carers, and students in the holistic management of clients with physical disabilities and cognitive impairment.
  • Supervise and mentor rehabilitation and healthcare assistants and physiotherapy students on clinical placements.
  • Contribute to service development and policies, providing specialist advice to clients and other professionals as required.
Key Responsibilities:
  • Deliver comprehensive holistic therapy assessments, setting short and long-term goals to maximize patient independence leading to safe discharge.
  • Provide specialist advice and support across the organization regarding the diagnosis and management of clients with multiple and complex disabilities.
  • Facilitate education and skills sharing among rehabilitation assistants and healthcare assistants.
  • Monitor accurate and comprehensive patient records in line with professional standards and policies.
  • Manage clinical risk within your own caseload and interpret clinical and non-clinical facts to form accurate diagnoses and prognoses.
  • Commitment to service development and strategic planning using specialist knowledge.
Requirements:
  • Experience working in a community setting.
  • A valid driving license and access to a car.
