As a key member of our team, you will play a vital role in providing support and services to carers, helping them manage their daily challenges and plan for the future.
Key Responsibilities- Provide tailored support and services to carers, including phone counselling, online coaching, and peer support
- Work with carers to develop support plans and provide ongoing support and guidance
- Collaborate with community care providers and other stakeholders to ensure seamless delivery of services
- Conduct intake and registration processes for carers, ensuring accurate and timely data collection
- Develop and maintain relationships with carers, community care providers, and other stakeholders
- Second language desirable
- Certificate IV in Disability, Community Care, or equivalent
- Knowledge of the community care sector and its challenges
- Proven ability to foster and maintain relationships with carers and community care providers
