Conwy Council Social Worker Role

We are seeking a dedicated and empathetic Social Worker to join our Older People & Hospital Team at Conwy Council. As a Social Worker, you will play a vital role in improving the lives of older adults and supporting them in their time of need.

Key Responsibilities:

  • Provide emotional support and guidance to older adults and their families
  • Assess and address the needs of older adults, ensuring they receive the necessary care and support
  • Work collaboratively with other healthcare professionals to develop and implement care plans
  • Conduct home visits and assessments to identify areas of need and develop strategies for improvement

Requirements:

  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Empathy and understanding of the needs of older adults
  • Ability to work in a fast-paced environment and prioritize tasks effectively
