About the Role

We are seeking a highly skilled and experienced Registered Nurse to join our team in the Discharge Lounge at Darlington Memorial Hospital or University Hospital of North Durham. As a key member of our team, you will play a vital role in ensuring the smooth and efficient discharge of patients from our hospitals.

Key Responsibilities
  • Assess patients' needs and develop, implement, and evaluate programmes of care.
  • Organise the delivery of nursing care, deploying staff effectively.
  • Communicate with the Wards, Doctors, Pharmacy, Relatives, Carers, and Transport and Patient Flow Team.
  • Supervise discharge lounge Health Care Assistants, liaise with the Discharge management team/ward coordinators and relatives.
  • Be responsible for patient care during their time in the discharge lounge and efficient coordination of booking patient transport and checking prescribed medication in readiness for safe discharge.
Requirements
  • NMC Registered.
  • Good interpersonal and communication skills.
  • Ability to work using own initiative.
  • Plan own workload and work effectively within a multi-disciplinary team.
  • Organisational abilities.
  • Computer skills.
