Job Description

We are seeking an experienced Community Nurse Case Manager to join our team at Leaders In Care Recruitment Ltd. As a key member of our multidisciplinary team, you will be responsible for conducting initial assessments in the community, training and assessing workers' competencies, and allocating support workers to patients. Your role will also involve collaborating with the wider MDT, attending meetings, and making referrals. Furthermore, you will manage a team of non-clinical case managers.

The successful candidate will have a strong background in complex care, including experience in delivering care to both adult and paediatric service users. They must also possess a car driver's license and be flexible to travel around the region. The estimated salary for this role is £47,000 per annum (based on 37.5 hours per week, Monday to Friday), which includes a £3,000 car allowance and mileage expenses.

Requirements

To be considered for this role, you must hold a current registration as a Registered General Nurse (RGN) or Registered Sick Children's Nurse (RSCN). You should also have advanced clinical skills in complex care, including ventilator management. Additionally, a minimum of 2 years' experience in a similar role is preferred.
